There are many ways.
The easiest is probably through this program:
Graphic Converter - http://www.flydragonsoft.com/
You can download the demo which should work to convert any images to black and white or grayscale. (Under the "colors" menu choose grayscale --> 2grays/ 16grays / 256grays

Another nice program that is free and easy to use would be iPhoto, which has various picture adjustments, including grayscale.
Grayscale is awesome.

You should never use the Black and White feature of your digital camera; you're loosing far too much quality. Try downloading some software like GIMP or, if you have it, use Photoshop. If you don't want/can't use either of those, MSPaint can convert to Black and White, although they literally mean Black and White, no grays.

Yes. When editing with a picture application, such as gimp or any software mentioned, you can make a layer with a black square that is 45% more or less transparent and then stretch it across the front of it.

Use photo editing softwares like Adobe Phtoshop CS or Ulead Photo Editor. Picasa can turn too.

You can use MS Paint.
Open up the image in this program and go to your menu bar,
then go to Images>Attributes
A dialog box will pop up and you should see an option to pick black and white or color.
